糖尿病足的治疗及进展

Progress in the treatment of diabetic foot

目的:对糖尿病足治疗的现状及进展进行综述,了解其治疗的有效措施.资料来源:应用计算机检索Medline 2003-01/2005-12期间的有关糖尿病足治疗的文献,检索词"diabetic foot,diet therapy,drug therapy,radiotherapy"等,限制语种为"English".同时计算机检索中国生物医学文献数据库CBM2003-01/2005-12期间的相关文献,检索词"糖尿病足,治疗",限定语种为中文.资料选择:选取包括具有神经、血管或感染情况的糖尿病足治疗的相关文献,进行初审,纳入随机对照临床试验、Meta分析和临床分析的文章.排除综述、个案报道.质量评价主要考察资料的真实性、严密性、实施过程是否严格及统计学处理的合理性.资料提炼:共收集义献91篇,纳入文献31篇.资料综合:糖尿病足是周围神经病变、缺血、足部畸形和感染共同作用的结果,是糖尿病发展后期的严重并发症,明确其发病机制对探讨疾病的发生发展有重要意义,诊断糖尿病足时应该明确其神经、血管、感染等情况.治疗方案也应根据溃疡的严重程度(Wagner分级)、血管状况、感染和神经病变的情况进行有针对性的治疗.结论:糖尿病足的短期效果通常比较理想,远期效果欠佳,只有明确糖尿病足的神经、血管、感染等情况,有针对性地治疗才能提高保肢率和降低死亡率.
