视网膜厚度分析仪在糖尿病视网膜病变检测中的价值

Application of retinal thickness analyzer in assessment of retinal thickness in diabetic retinopathy

目的 目的观察视网膜厚度分析仪(retinal thickness analyzer,RTA)在诊断和量化糖尿病视网膜病变(diabetic retinopathy,DRP)中糖尿病黄斑水肿(diabetic macular edema,DME)的应用价值.方法 应用RTA对147只DRP眼进行后极部视网膜厚度分析,并与38只年龄匹配正常对照眼进行比较.分析病程和血糖与RTA检查结果的相关性.结果 1.RTA可发现视网膜的早期亚临床状态下的视网膜增厚.后极部平均厚度(PPAT)和中心凹旁平均厚度(PFAT)与DRP病程呈正相关,可能对早期判断病变发展进程具有提示作用,对预测DME的进展和预后具有重要的参考价值.2.中心凹平均厚度(FAT)与DRP病程相关系数最大,DME的发生是从中心凹周围向中心凹发展.中心凹平均厚度的增加呈现逐渐递增的趋势,它的改变提示病变发展的进程.病程和血糖与FAT呈正相关.3.视网膜平均厚度在早期就开始变化,而最大厚度的变化不敏感,说明DRP开始可能是弥散的病变,晚期会出现局限的高度水肿变化.结论 RTA为观察DRP和DME病程发展、疗效评估提供了一种有价值的手段.
